Requesting Your Health Records

You have the right to request access to or copies of your health records. To request your records, log in to the Sessions Health Client Portal and send a secure message to your provider. If you are unable to access the Client Portal, please contact The Safe State Counseling, PLLC for assistance.

Patient Privacy Complaints

If you believe your protected health information has been or may have been used or disclosed in violation of HIPAA or the Texas Medical Records Privacy Act, you may file a complaint with the Texas Attorney General's Consumer Protection Division.
